I consent to the transfer of my personal data from the Neko Health Medical to Neko Health for the purpose of Neko Health using it as described below to further develop its devices and technology to better provide preventative healthcare.

Neko Health AB (“Neko Health,” “we,” or “us”) is a Sweden-based company and the parent of the Neko Health group. Our affiliated professional entity, Neko Health P.A. dba Neko Medical Health (“Neko Health Medical”), operates private healthcare clinics in the US (each a “Clinic” and together the “Clinics”). We are committed to protecting your personal data.

If you consent to us processing your personal data, including special categories of data such as health data, Neko Health Medical will share such data with Neko Health, for the purpose of developing safer, more efficient, more innovative, and higher-quality healthcare experiences.

Neko Health Medical is responsible for the processing of your personal data, including health data, when you visit one of its Clinics for the purposes of carrying out, administering, and assessing the quality of the health checks performed using the technology and equipment offered to you at the Clinics.

With your consent, Neko Health Medical will share your information with Neko Health so that we can use your information along with information we already hold (such as Ask Neko AI chat data) to test and develop our tools, devices and technology to better provide preventative healthcare. To the extent this processing is considered scientific human subjects research, we will obtain separate consent where required and comply with applicable human subjects research laws.

We will pseudonymize your personal data (i.e. replace direct identifiers, such as your first and last name, with artificial identifiers) to the extent feasible and take steps to ensure that pictures of you do not appear together with any other information which may identify you directly. With your consent, we will process your image data, demographic data, physical attributes, health data, biometric data, identity data, communication data, data in your patient records, and information about your lifestyle. All personal data is collected directly from you in connection with your healthcare visit.

If you do not consent, we will not process your personal data as described in this form. You may withdraw consent at any time by updating your settings in the patient application or contacting us. Withdrawing consent does not affect the processing based on a previously given consent but we will stop future processing for this purpose.
